Appointments
All of our physicians are currently accepting new patients. Please call us at 540-373-1331 to make an appointment.

Federal healthcare regulations now require us to convert our paper records to electronic health records. In order to achieve this monumental task, we will need your cooperation to ensure that your record contains the most up-to-date information. Please print out and complete these patients forms prior to your next appointment:
Please arrive 15 minutes early for your appointment and bring your current insurance card(s) and a photo ID. It is also helpful to have a written list of any questions you would like to discuss with your physician.
Cancellations
Your appointment time is reserved exclusively for you. Twenty-four hours notice is required for cancellation of appointments to avoid a $25.00 missed appointment fee. Thank You.
Late Arrival Policy
We believe that our patients' time is valuable. As a courtesy to our other patients, those who arrive more than 15 minutes after the scheduled appointment time will be rescheduled. In some cases, if openings are available, it may be possible to reschedule for the same day at a later time. Advanced notice of your late arrival is necessary for any special allowances or considerations.

Your request will be processed within 24 hours, during normal business hours, Monday – Friday, 8:30 am – 4:30 pm, excluding holidays. If you do not receive confirmation of your request within 48 hours, please contact our office at 540-373-1331.
New Patients
Please print out and complete the Patient Information and Medical History form below, and bring them with you to your first appointment. It is also necessary for you to bring in your current insurance card(s) and a photo ID.

Medical Records
To request a copy of your medical records for yourself or another physician, please print out and complete the Medical Records Release form above, then fax it to 540-373-1124. Note: There is a fee for copies that are requested for personal use.
Referring Physicians Only
To request a consult or diagnostic testing for your patient, please fax a completed appointment request form to 540-373-1124. Include a copy of the patient’s last office note, current EKG, and recent labs if available. We will contact you when the appointment has been scheduled. You may download the form below or call us at 540-373-1331 to have forms sent to you.
